Open Enrollment

*Open Enrollment 2020/2021 begins July 27th, 2020 at 8:00am and closes Aug. 13, 2020 at 4:00 PM*

Active Employees enrolled in Flex and Dependent Daycare accounts must re-enroll each plan year to continue the benefit.

All other Employees and Retirees who wish to keep their current benefits do not need to log-in to SmartBen. Your elections will automatically roll forward.

Employees and Retirees who carry their spouse on one of the FHSD MEDICAL plans are required to submit a Spousal Exclusion Form each plan year. See step 6 below.

If you get disconnected from this webpage, you can access it again by visiting and clicking Human Resources > Benefits > Open Enrollment.

Please review the documents in the following order:

  1. Review the above information in green text.
  2. Review the Open Enrollment Book that applies to you.
  3. Review the Premium Cost Sheets below, which detail the cost breakdown of your benefit options.
  4. Review Instructions on Enrolling in SmartBen
    • Reminder: print and review your confirmation page, retain for your records.
  5. IF you are an EMPLOYEE and you NEWLY elected the Consumer Driven Health Plan for Medical, complete the HSA Application and send in to the Benefits Office prior to the end of Open Enrollment.
  6. Employees and Retirees who carry their spouse on FHSD MEDICAL plan must complete the Spousal Exclusion Form and send in to the Benefits Office prior to the end of Open Enrollment.
  7. Employees who elected supplemental life amounts greater than the guaranteed issue amounts (see page 26 of the open enrollment book) are required to submit an Evidence of Insurability Form. Once the form is complete submit it one of the following methods:
