*Please keep in mind that even though your Skyward Employee ID # includes a 1 at the beginning, your SmartBen log in is still formatted using your 5 digit employee ID #*

Effective 10/1/2022 the changes listed below will be in effect:

District Contribution:
- Increase of 3.9%

Medical:
- 3.9% increase in premiums
- Elimination of the High Plan

Dental:
Aetna Dental:
- Increase in annual max to $1,750

Family Dental Services:
- 3% increase in premiums

- 10% increase on services that require a lab fee

Cigna Dental:
If choosing one of the Cigna DHMO plans, please note that it may be difficult to find an in-network dentist and the network may change during the plan year.

Vision:
- 1.35% decrease in premiums
- Enhanced benefit with Eye360

Life/Disability:
- Move from MetLife to Lincoln Financial
- Increase/enroll in employee voluntary life up to $200,000 without proof of insurability
- Increase/enroll in spouse voluntary life up to $50,000 without proof of insurability

    • Reminder: print and review your confirmation page, retain for your records.

  6. Employees and Retirees who carry their spouse on FHSD MEDICAL plan must complete the Spousal Exclusion Form and send in to the Benefits Office prior to the end of Open Enrollment. Both benefit eligible employees need to complete the spousal exclusion form and check the “spouse works for the district” box.
  7. Employees who are currently enrolled in supplemental employee during 2022-2023 Open Enrollment period. This enrollment is considered a "true open enrollment" meaning any employee, whether previously electing coverage or not, can elect up to the Guaranteed Issue Of $200,000 without providing Evidence of Insurability Form. Elections exceeding the $200,000 Guaranteed Issue Amount will require Evidence of Insurability. Visit the My Lincoln Portal and log on using the code FHSD-EE. Lincoln Financial Group evidence of insurability forms must be submitted electronically in order to be considered.

Thursday, August 4 | 9:00 - 11:30 AM
Friday, August 5 | 10:00 AM - 12:30 PM
Tuesday, August 9 | 5:00 - 6:30 PM

Important:
This is a passive enrollment. If you don’t do anything, your current benefits will roll over into the next plan year at the new rates. 
 
If you are enrolled in the High plan and do not do anything, you will be automatically enrolled in the Base plan for the new plan year.
