Eligibility Requirements
|
Healthcare enrollment requires
support staff to work at least 30 hours per week and at least 174 days per
year. Certified staff are eligible for part time healthcare benefits by
working at least 17 hours per week and full time healthcare benefits by
working at least 30 hours per week. Retirement enrollment requires that a
non-certified employee work at least 20 hours per week on a regular basis in
a position that requires the employee to work at least 600 hours during the
school term. A certified employee must work at least 17 hours per week on a
regular basis in a position that requires the employee to work at least 600
hours during the school term.

Medical
|
All eligible employees have the
option of enrolling in one of three medical plans. All three plans utilize
the UHC Options PPO Network of
providers, www.umr.com. Eligible employees can find additional

Dental
|
All eligible
employees have the option of enrolling in one of three dental plans: Essex
Dental, Humana, or Family Dental Service, Ltd. The plans offer a variety of
providers and benefit levels. Eligible employees can find additional

|
|
Vision
|
All eligible
employees have the option of enrolling in the vision plan offered by EyeMed
Vision Care. Benefits include exam and lens coverage once every 12 months and
frame coverage once every 12 months. Eligible employees can find

|
|
|
District Contribution
|
Each eligible full time
employee receives a monthly credit that ranges from $465 to $868 to be used
to offset the medical/dental/vision premium. Each eligible part time
employee receives a monthly credit that ranges from $232.50 to $434.00 to be
used to offset the medical/dental/vision premium.

|
|
|
Section 125
|
All eligible employees may
elect to have their healthcare premium withheld pretax. In addition,
employees may enroll in a Dependent Care Reimbursement Account, a Healthcare
Flexible Spending Account, &/or an Adoption Assistance Reimbursement
Account. If an employee is enrolled in the high deductible medical
plan, he/she may elect to contribute to a Health Savings Account.

|
Life
|
All eligible employees are
enrolled in $75,000 term life insurance coverage. In addition, employees are
eligible to apply and pay for supplemental term life insurance on themselves,
their spouses, &/or their dependent children.
|
|
|
|
|
Disability
|
All eligible employees will be
enrolled in short term disability coverage. This coverage may provide up to 9
weeks of 70% wage replacement in the event of a short term disability.
Additionally, employees may elect voluntary long term disability coverage that
may provide 60% wage replacement coverage in the event the disability
continues beyond 9 weeks.

|
Retirement
|
All eligible employees are
enrolled in a statewide retirement plan. Benefits are determined by years of
service, age at retirement, and the employee’s three highest consecutive
annual salaries.

*Employee Assistance Program
|
All employees are eligible to
participate in the FHSD Employee Assistance Program. This is a confidential
service provided by licensed counselors. A wide range of assistance programs
are offered including relationship counseling, stress counseling, mental
health counseling, eldercare advice, financial and legal counseling, etc.

|
|
|
|
*Wellness Program
|
Currently offering annual flu
shots, annual access to an onsite mammography van, **hepatitis immunization,
on-site screenings for cholesterol/blood sugar, annual health fair, blood
pressure screenings, and employee wellness incentive program.
|
|
|
|
|
*Tax Sheltered Annuities
|
All employees are eligible to
participate in this supplemental voluntary retirement pretax payroll
deduction savings plan. There are numerous brokers and products to choose
from.

|
|
|
Worker’s Compensation
|
Francis Howell School District
is a member of a self-insurance pool, Missouri United School Insurance
Council, which provides Workers’ Compensation coverage with the state of
Missouri. The claims are administered by Gallagher Bassett Services on behalf
of Francis Howell. Workers’ Compensation is basically no fault insurance for
injuries that are sustained while you are within the course and scope of your
employment, subject to applicable Missouri law. Benefits for injuries that
are found to be compensable include payment for medical treatment, payments
for lost wages, and if applicable, payments for permanent handicaps.

Paid Sick Leave
|
Regular and part-time benefits
eligible employees (as defined in Regulation 4510 CR, LO, FD, ES and SU)
shall receive ten (10) sick leave days per school year. Part-time benefits
eligible employees working 20 hours per week (4 hours per day) will receive
4.50 sick days. Please see Board of Education Regulation 4320 GE for more
information.
Certified employees may
participate in a sick leave bank. The purpose of this bank is to provide
extended paid sick leave to certified employees who have exhausted all paid
sick leave, vacation, non-contract days, and District-provided short-term
disability leave, and who are required to continue to be absent due to their
own serious illness or injury, or the serious illness or injury of their
immediate family, as defined under this policy. Please see Board of Education

Paid Personal Days
|
Benefits eligible employees
working more than 4 hours per day may take up to three (3) personal days with
pay per school year to be deducted from their sick leave allowance or from
the accumulated days in their sick day account if their annual sick day
allowance has been depleted. Please see Board of Education Regulation 4320 GE

Name Change Procedure
|
Employees who experience a name
change must make an appointment with the FHSD Benefits Office to complete all
necessary paperwork. At the time of the appointment, the employee must have
either a new Social Security card or a receipt from the Social Security
Office showing application for a new card has been made. Employee name
changes MUST match the employee’s current Social Security card.
